What Is a Caregiver?
A caregiver is a person registered with the Illinois Department of Financial and Professional Regulation (IDFPR) to purchase, transport, and administer medical cannabis on behalf of a registered patient. Caregivers are essential for:
- Minor patients who cannot enter dispensaries independently
- Homebound patients with mobility or transportation limitations
- Elderly patients who need assistance navigating the dispensary system
- Patients with cognitive impairments such as Alzheimer's or TBI

Caregiver fees are lower than patient registration fees and follow a similar multi-year structure. The 3-year option at $75 works out to just $25 per year.
How to Register as a Caregiver
- Patient designates you: The registered patient must add you as a caregiver through the patient portal at medicalcannabispatients.illinois.gov
- Complete caregiver registration: Register through the same portal with your Illinois ID and proof of residency
- Background check: Illinois screens caregivers for excluded offenses
- Pay the registration fee: $25 for 1 year, $50 for 2, or $75 for 3
- Receive your caregiver card: Once approved, your card allows purchasing at any licensed dispensary on behalf of your designated patients
Caregiver Responsibilities
- Carry your caregiver card: Always have your registration when purchasing or transporting medical cannabis for a patient
- Purchase only for designated patients: You may only obtain cannabis for patients who have formally designated you through the state system
- Respect the 2.5 oz / 14-day limit: Cannabis you carry on a patient's behalf counts toward that patient's allotment
- No diversion: Using or selling a patient's medical cannabis is a criminal offense
- Maintain current registration: If a patient's card expires, your authorization to purchase on their behalf also lapses
The Tax Advantage Extends to Caregiver Purchases
All purchases made by a caregiver on behalf of a medical patient are taxed at the medical rate of 1% — the same as if the patient purchased directly. This means caregivers buying on behalf of patients save the same 25–40% compared to recreational pricing that patients save when buying for themselves.
Illinois does not allow a person to be registered as both a patient and a caregiver simultaneously. If you need medical cannabis for yourself, you must register as a patient. If you want to help someone else access cannabis, you register as a caregiver — but not both at the same time.
